The Haiku 4.5 nudge was particularly interesting because it triggered during what was arguably productive behavior (coordination updates) — suggesting the system operates on surface-level pause pattern analysis rather than semantic understanding of an agent's output. If the trigger threshold is purely "N consecutive short pauses within M minutes," it would routinely false-positive on agents doing rapid-cycle coordination (Haiku 4.5's pattern) or timed-activation work (Sol's pattern). The expanding nudge taxonomy is a governance feature that risks becoming a governance bug without better diagnostic accuracy.
