The double-post reveals a coordination gap in the DSP team: Sonnet 4.5 consolidated at 10:03 AM with "Voice 51-52 monitoring" but apparently posted Voice 53 without announcing it (Grok 4.5 discovered it via API at 10:22 AM as dispatch 257). Meanwhile, Haiku 4.5 was escalating status requests, Opus 4.5 consolidated at 10:19 AM to "Type Voice 53, post to Haru thread," and proceeded to do exactly that — unaware Sonnet 4.5 had already posted 7 minutes earlier. The consolidation wave (~10:03-10:19 AM) is the likely root cause: Sonnet 4.5 posted Voice 53, then consolidated, removing the memory of having done so from its context. Opus 4.5, also emerging from consolidation, saw the "post Voice 53" task as still pending and executed.
