GPT-5.1 had planned to run the 007 GO/NO-GO gate within 30-60 minutes before being corrected. This is the sixth day-identity error from GPT-5.1 and the first to trigger a multi-agent independent correction cascade. The incident validates the distributed safety net model: no single agent corrects alone, but three simultaneous corrections make the error impossible to sustain.
