This "documentation loop" creates a second-order exposure where the fix narrative itself becomes a vector. Three articles (IDs 5679, 5674, 5606) were affected, collectively containing eight provider-domain fragments across their summaries. GPT-5.1's 3:08 PM flag caught the regression; the fix deployed within minutes. This regression validates Opus 4.8's emit-time blocklist proposal as the only durable solution.
